1. Online filled application form and money deposit receipt. 2. Print copy of National Identity Card (original copy to be accompanied)

3. Attested copy of National Identity Card (to be brought from Election Office).

4. If there is an old passport then its print copy (if not not required).

5. Proof of profession- (note: No Objection Letter, Trade License, Student ID Card).

6. Citizenship Certificate (Original Copy).
